What Actually Happens Inside Your Skin During Microneedling

The core idea behind microneedling is controlled wounding. The device creates thousands of microscopic channels in the surface of your skin, each one triggering your body’s natural healing response.

That healing response is the whole point. When your skin detects an injury, it floods the area with collagen, elastin, and growth factors to repair it. Microneedling hijacks that process in the best possible way, creating just enough ‘injury’ to trigger a significant healing and renewal response, without actual damage.

Your skin doesn’t know it’s being tricked. It just knows it needs to rebuild. And rebuilding is exactly what makes your skin look better.

Step 1: Before the Treatment (The Numbing Phase)

When you arrive at our clinic in Temecula, your skin is cleansed, and a topical numbing cream is applied and left on for 10 minutes.

While the numbing sets in, your provider will discuss your skin concerns and the needle depth for your session. Different depths are used for different concerns: shallower for fine lines and glow, deeper for acne scarring and texture.

Once you’re numb, the cream is removed and your skin is prepped with a clean, sterile serum.

Step 2: During the Treatment (What You’ll Actually Feel)

The microneedling device, the Candela Exceed, moves across your face in small, overlapping passes. Most people describe the sensation as a mild scratching or vibrating feeling, like light sandpaper. Some areas are more sensitive than others (the forehead and upper lip tend to be most noticeable), but with proper numbing it’s very manageable.

The treatment itself takes 30–45 minutes for a full face. Your skin will look pink, like a mild to moderate sunburn, by the time it’s done.

Step 3: Immediately After (Hours 0–24)

This is the phase that surprises people the most. Your face will be red, slightly swollen, and warm to the touch. You’ll want to skip makeup for the rest of the day (your skin needs to breathe), avoid sweating or heat for 72 hours, and apply only gentle, fragrance-free moisturizer and SPF if you go outside. No active skincare: no retinol, vitamin C, acids, or exfoliants for at least 72 hours.

Step 4: Day 2–3 (The Healing Cascade Begins)

Your body is now beginning a cascade of healing activity. Platelets rush to the micro-injury sites and release growth factors, including PDGF, TGF-β, and VEGF, that signal fibroblasts (the cells responsible for producing collagen and elastin) to activate.

You may notice some dryness or mild flaking as the surface layer of skin renews itself. This is normal and temporary. Keep moisturizing. Keep wearing SPF.

Step 5: Weeks 1–4 (The Collagen Remodeling Phase)

Over the following 4–6 weeks, your skin is actively laying down new collagen fibers and reorganizing existing ones. The technical term is collagen remodeling, and it’s responsible for the gradual, progressive improvement you see after each session.

Your skin texture smooths out. Fine lines soften. If you had acne scars, they start to fill in. Pores appear smaller. Your overall skin tone becomes more even.

Most people notice a visible difference about 2–4 weeks after their first session. The results continue to improve with each subsequent treatment.

How Many Sessions Do You Need?

For general skin improvement, glow, and maintenance: 3 sessions spaced 4–6 weeks apart is a great starting point. Results build with each session.

For acne scarring or more significant texture concerns: 4–6 sessions is typically recommended, and some clients continue with quarterly maintenance after that.
